Case Management

About Case Management

The Case Management program offers comprehensive supportive services, advocacy, and resource and referral to individuals and families within the community.

The advocate will assist and meet with each individual to identify how we can support and provide the client with the information and services they need. 

  • Wraparound services
  • Referrals for healthcare and behavioral health services
  • Referrals for basic needs assistance
  • Housing assistance
  • On-going support through phone calls and in-person visits
  • Helping clients navigate obtaining services by accompanying clients and their families to meetings 
  • Support services such as assistance applying for a Temporary Protection Order (TPO), SNAP, TANF, Medicare, Medicaid, obtaining bus passes, clothing, housing, food, and more.
